Early
Childhood Connections in conjunction with the Project ECHO Screening
and Evaluation Program offers in-depth developmental evaluations
using certified therapists and early childhood special educators for
children less than three years of age if a problem in development in
suspected.

A
Service Coordinator helps the family assess their resources and
identify needed services such as therapy or Family Support Services.

The
Individual Family Services Plan identifies the child's needs and the
family's priorities for their child.

The
plan is reviewed at least every six months.

Together
the family and Service Provider develop a Transition Plan to prepare
for a change from infant/toddler services to preschool special
education services.
